  1. In the United States, your mother's maiden name is the surname of her father.


  2. Her family name, before marriage.  It should be shown on your birth certificate.. and if you know her parents, it is probably the same as her fathers.  It will be different, if her mother is divorced and remarried.

    One does not say that a woman who didn't marry had a 'maiden' name.   One only has a 'maiden' name after one is married....strange, that.
