  1. It is called a 2.1 mm chassis mount dc power jack. The  link is to exactly what you are asking for, it's about half way down the page.

    The model 31-155-0 has a built in switch which will interupt battery power when you plug into it.  the model 31-133-0 is the same thing without the built in switch.

  3. You're probably thinking of a coaxial power jack or connector.  Round with a pin in the center.
