You may go to any hospital or clinic to get your "statement of fitness". it will have to be signed and stamped. you will need your blood type : blood donor card

while in Qatar , you will undergo HIV test , the country does not accept test results from foreign countries

I think its five years but you have to renew it every month unless you are so wealthy its unbelievable. Why dont u jus buy a qatar instead.
