In a car crash, forces are transmittted to the bodies of the occupants. With a broad belt these forces are spread more evenly over the surface of the body and so do less damage. In other words the PRESSURE on the body is less.

Think of being poked with a needle and with a baseball bat with the same force. The needle does more damage.

The needle corresponds to a narrow belt and the baseball bat to a broad bat.
