According to the National Center for Health Statistics, the 50th percentile (half are taller/heavier, half are shorter/lighter) for 15-year-old girls is approximately (the chart is a bit hard to read on-screen):

5' 4"

114 lbs

Please note that "average" does not equal "good" or "normal" in this area. Except for severe obesity or severe malnutrition/underweight issues, a very wide range of heights and weights is perfectly normal.

I couldn't find reliable statistics for the other three measurements. Clothing size is going to be especially difficult since each manufacturer may use slightly different "standards" when making their own "Junior 4" size or what have you.
