as an md you have to make a choice... you can go into private practice and bill your patients directly whatever the market will bare or you can practice in the healthcare system which is a single payor type system where fees are pre-set and you bill the government for your work.  You cannot do both.

Average salary ranges widely from $150K per year for a low end GP to $500K per year for a busy speciality in Psych.
