Employee? or Contractor?  Employee: $8.00 per hour plus commission incentives for productivity.  Contractor: 60/40 Therapist/Establishment split on revenue earned.  If employee, pay should be for a block of time, not only when the mt has an appt.  I've seen several scenarios, and these seem to be the ones that last the longest, especially if it is a start-up business or service.
