Ken Griffey Jr's Winning Run for Super Nintendo. I realize that my choice is completely indefensible, as it wasn't a particularly great game by any objective standard, but I grew up playing it and I love that game.

I liked bases loaded 4 for regular NES.  It was my favorite and easy enough to play.

For the game that combines realistic graphics and simulation gameplay, it would have to be MLB The Show 08.

The most innovating baseball game of all time is Baseball Stars, a NES 1989 product. It is legendary that it is one of the first (if not first) sports game that allowed you to save and create personal teams, hire and trade players, pick up free agents, create a player, and give players training to upgrade their stats. The defensive play was unparalleled to games at the time, when fielding was near automatic. You could do triple plays, steal bases, record saves, have 10 run innings, or even throw a perfect game. Without this, modern sport games of todays standards would probably be pushed back a few years.
