I use Spybot Search and Destroy - it's free (I beleive you can donate if you wish) and not only does it have a spyware search facility, but it also blocks threats in your web browsers and hosts files, and it also has something called "Teatimer" which monitors programs and queries you if programs try to change registry entries, etc.  You have to hunt around a little to find this and turn it on, and you may thing it gets a bit annoying when it flags up legitimate registry changes during installations, updates, etc, but I would always have it turned on.
