I second Il Cortile.  Best italian place in little italy.

I also like The Grotto on Forsyth, it's a little off the beaten track and hidden, but it'll give you a real new york feel.

Little Italy is ok, but if you want better, try IL Mulino at 86 West Third Street, between Sullivan and Thompson Sts. Great food and service.

I try to eat in Villa Mosconi every week. No place is more welcoming. And the food is amazing. This is the one Italian restaurant that we recommend to all of our friends.
