The Cigar Bar, of course.   Down Montgomery in the Jackson Square/Barbary Coast historical district, past the Bubble Lounge and Boccadillo, near Bix.

The voodoo lounge on Van Ness and Lombard is a good spot.  They actually found a loophole in the no smoking laws in California so you can smoke at the bar.
