I'm an American who has rented a car in Italy many times.  I find the best rates for 1 week or longer rentals is through www.autoeurope.com or www.1800flyeurope.com.  They don't charge extra for one-way rentals as long as you pick up the car in Italy and return in Italy.  Their rates include full insurance - the type that even if you total the car, you are fully covered without a deductible.  They also charge you in US Dollars rather than Euros so you don't have to worry about the fluctuating exchange rate and that "hidden" 3% foreign transaction charge on your credit card.

    For 1 and 2 day rentals, try easycarspa.com.  The site is in Italian but they do have an English translation of their terms and conditions.

    You'll notice that car rentals in Italy are generally higher than most other countries.  This is because collision insurance and theft prevention are MANDATORY.  The rental car coverage that is included on most U.S. credit cards doesn't work in Italy, like it does in other countries.  When you are getting your quotes, check the deductible on the coverages that you are getting.

    When traveling to Italy, a car is best way to see the real Italy - the Italy outside of Rome, Pisa, Florence, Venice, and Milan.  Enjoy!
