  1. Traditional Weddings are held in a church.

    I am not sure what else you want to know there really isn't any "criteria" for a "formal" wedding aside from "formal" attire.

  2. The dress should be formal, of a formal material, long and something that you're happy with and feel good in.

    A formal wedding should have 8-10 bridesmaids and likewise, groomsmen.

    Tuxedos are of course part of a formal wedding. Black and traditional are great. Tails are acceptable in VERY formal settings and "white tie" is the most formal of all.

    Flowers should be plentiful - lining the aisles, large alter arrangements, full bouquets, tasteful and beautiful centerpieces etc.

    Formal weddings are most always at night.

    Dinner should be plated, drinks should be hosted (free to the guests), a champagne toast is practically required by law. A live band is more formal than a DJ. A string quartet is essential for the formal ceremony and cocktail hour.

    and most of all NO MONEY DANCE!!!!

    not sure what you mean by etc. but it should be a sit down dinner not a buffet.  There should be a cocktail hour but it can be a cash bar plus you should serve (free) champagne. The cake should be ultra gourmet - and beautifully decorated and a center of attraction.  There should be centerpieces on evry table.  The church should have flowers at the altar and on every other pew.  Your should have a dance after dinner - not a DJ, but a band.  Never go past midnight.

    TIME:  Usually after 6pm, but there is no set time.

    TRANSPORTATION: Drive away in a vintage Rolls Royce, use black stretch limousines for the rest of the day.

    You do not have to indicate black tie invited or anything on the invitation. Sending a formal invitation with engraved lettering on heavy stationery with all the inner envelopes and acoutrements will send the message for you.

    As will the venue for a formal reception: country club; museum; etc. rather than a park or VFW.
