  1. There is more to an acknowledgment form. A jurat is simply a certification attesting to the fact that someone signed and swore to a given statement or set of facts. In an acknowledgment form, the officer is certifying not only that the person who signed the document personally appeared before him and acknowledged to him that the same is true and correct but that it is done on his own free will.
