They're usually the daughters of high ranking nobles such as counts, knights, and earls. For example Anne Boleyn was the daughter of Sir Thomas Boleyn and was lady in waiting to Henry the VIII's wife Catherine. Catherine Howard was Lady in Waiting for his wife after Anne Boelyn Jane Seymour. Diana, Princess of Wales' ladies in waiting were her two sisters.

    I found that reading biographies about the royals quite helpful.The Queen Mother's ladies-in-waiting included Diana's grandmother Ruth, Lady Fermoy.

    As a precision, a lady in waiting's duties usually are ceremonial, and are about the "higher" aspects of attending the higher rank noblewoman at whose court she resides. While she's often housed, fed and clothed by her mistress, those are not considered wages, and the duties of a lady in waiting are regarded as a female counterpart of the Noble Service owned by a vassal to his Lord.

    Servants, on the other hand, are just paid menials.
