  3. I would think the leading cause of premature death would have been the communicable diseases. Depending what part of the world, and what part of the country, I'm sure cholera would have been right up there but lots of people died of influenza, pneumonia, measles, diphtheria, scarlet fever and all those diseases that have now been virtually eradicated or are treatable with antibiotics.
