Humbly, by recollection, one of the last medical  meetings I went to discussed this topic.  If memory serves me :

Heart Disease followed respectively by Cancer, Diseases related to diabetes and then processes related to Alzheimer's ( 70% of people who have dementia have alzheimer's disease- the remaining percentages are related to other brain disease : Parkinson's diseases, Pick's disease- a small percent, Lewey bodies disease and vascular causes- strokes, mini strokes etc.
