  1. It depends on what the instrument is.  That will tell you where to file it.  Real Estate, recorder of deeds, a will, probate court, etc.


  2. In the US, file it with the County Recorder (California), Prothonotary (Pennsylvania), or other legal entity in the place of record.  Obviously it varies by location.
