  2. Diane gave you the link to the Standards of Care (SOC).

    In general you have to be 18 before surgery.  I say "generally" because some surgeons specify you must be legally an adult in your home country.  You also have to have been living full-time 24/7/365 in your identified gender for at least one year.  You also have to have been on HRT for at least 6 months.  Lastly you need 2 letters of reccomendation (one from a therapist and one from a PhD or psychiatrist).  It's all in the SOC.

    For Hormone Replacement Therapy (HRT) you have to be a minimum of 16 according to the SOC.  However, "blockers" can be started earlier.

  3. You ARE in the wrong category - this question would get more, and better, answers in LGB&T ( Home > Society & Culture > Cultures & Groups > L*****n, g*y, Bisexual, and Transgendered ), but nonetheless;

    Neither of these treatments will be offered to anyone who is under 18 years old. There is no formal upper age limit, but some clinics/surgeons set their own.

    The other requirements you're asking about are set out in the HBIGDA Standards of Care.

    You can find a copy of these here; http://wpath.org/Documents2/socv6.pdf

    And more information about them, and their application, here; http://www.tsroadmap.com/reality/hbigda-...
