What is the name of this hanging plant?

Question

It has small leaves with thin stems that grow pretty long. The flowers are either pink, pink and white, or purple (5 petals). Each flower is about the size of a dime, and are clustered about 20 small flowers to a bunch. I have kind of adopted it from my girlfriend, and would love for it to flourish.
