hello.. hope i have find some answer in my question.. is medical certificate.. really needed in passport processing in the DFA...

Are you Filipino? If you are you only need to be there, fill-up an application form and bring your:

Birth Certificate from NSO or a Certified True Copy of the birth certificate

3 passport size colored photos with royal blue background. you should wear a collar and make sure that at least 3 cm of your face is there. They actually measure. Probably the safest way is just dress your best and there are available photolabs inside DFA.  

Proof of identity indicating full name, date and place of birth and citizenship. It could be either of the following:

.Marriage Contract

.Voter’s Registration Record

.PRC / IBP ID

.Baptismal Certificate

.Land Title

.Seaman’s Book

.Driver’s License

.Income Tax Return (Old)

.Government Service Record

.Old SSS-E1 form/or digitized SSS-ID

.School Form 137 or Transcript of Records
