there was a procedure to follow in design ( engineering and other fields ) and it was a recognised procedure to follow in engineering , the older guys would remember it perhaps ,and it was a critical analysis designed to detect in built errors , also errors are defined as mathematical , human , physical (site) and other categories , it might be worth defining the errors and see if quality procedures were executed . good luck . don't know any site but it should be existing procedure .
