What is the process of changing your entire identity?

Question

Virtually changing everything possible about you. What is everything that can legally be changed (ie name) and such. How do you go about doing so? Thanks.

Note: Please do not ask my reason for wanting to know.

You can change your birth name (but the new name will be registered that is was changed from A to B), have full plastic surgery to look differently, gain/lose weight.

Now days, it's very difficult to accomplish successfully, legally or even illegally. Changing your actual name doesn't amount to anything really...
