  1. The top of the flange should be flush with the finish floor.

If it is too high, the toilet won't seat properly and it will rock.

If it is too low, there are thicker wax rings you can use to make up the difference.

  2. the toilet flange should set on top of the finished floor . useing multiple and thicker wax rings is not the proper way to go  . If you can raise the flange the distance needed ,that would be the best way to go . they also make flange extension kits if you are in a jam .These will help you bring up the flange to the proper height. Also if the flange is too high you can make the height difference out of  plaster of paris ( work fast )or even wood ,using the bottom of the toilet for a template so it kind of blends in .the wood can be painted too .

  3. Flush is fine. Using multiple wax seals can cause problems since it is possible for them to slide when installing the toilet and block the discharge. You can purchase thicker wax seals.

  4. it is flush with the finish floor-not sub floor

  5. If flush with finished floor is fine, even if 3/8 inch above is fine. If flush just use a wax ring with the plastic collar, if this doesn't seem to be enough use a second ring without the collar. If there hasn't been any problems in the past, use the same as was originally.
