  1. A major work, such as a painting, sculpture, novel, album, play, or opera, should be italicized (or underlined, if italics are unavailable).

A lesser work, such as a song, poem, or short story, should be enclosed in double quotation marks.

  2. Formatting is a thing of choice really. It depends on the context you wish to do it. Most cases, either italicising, underlining or using quotes will work just fine as long as you stay consistent.

However, with works of art, italics tend to be used, especially when working in the MLA format, which is the standard format of choice within the American academic community. Harvard Style, which is used more internationally, also prefers Italics.

However, if you are writing this for a non-academic source, say a personal research paper for an article or some such, it's all personal choice on what you prefer.

  3. well, i'm not sure what you are trying to work on, if it is a paper for school or something for yourself.  if it's for school, your teacher should tell you what style to use (apa, chicago, etc.) and  if you look in a handbook for those it should tell you somewhere how to do it; if it is just for you, do anything you want to distinguish it from the rest of the sentence.  the important thing is to stay consistent.  so if you use italics, use them throughout.  if you underline, underline throughout.  etc.

  4. It could be any one or all of them because it is a title.
