  1. The red rectangular one with 9 pegs on top and 3 octagon holes in the bottom. It makes it impossible to build with it.

  2. Probably the red prototype pieces Lego makes... The Red Ghost (seen at http://www.lucajuventino.altervista.org/... for instance, is rumored to have only had only 20 or so ever made.

  3. That's a pretty complicated question, actually.  Technically, I think the rarest would be the 24 carat gold Bionicle mask of which there was only 1 ever made.  I believe it was given to one of the owners of BZPower.com, but was later auctioned on eBay.  I forget how much it sold for, but quite a bit.  There's also 5 copies of the gold C-3P0's that were made (not sure if they were 24 carat), which were given away as prizes in a contest by Lego in 2007.

As mentioned in the previous answer above, there are prototype pieces of which only a few are made-- they're never released in sets, but a few can be found floating around in Lego collector's possessions as well as Legoland theme parks and various other places.  There are MANY different types of these elements, but there aren't many of each one.  A few examples are the red ghost (a commonly known one), but also things like the "aqua man", transparent skeletal horse, trans-yellow rock monster, red train tracks, etc.

Otherwise, there are many rare Lego elements that were only released in a single set, or were released so long ago that they're now very rare.  Good examples are the white pigtail hair element, granulate trees and bushes, the 50x50 baseplate, whistle-controlled trains, the old gray 2x2x3 corner slope, etc.
