  1. No he does not have to be eligable,as he can only field, can't bat or bowl or wicket keep or be a runner for a batsman

  2. Do you mean the twelth man, or the substitute fielder?

The twelth man, as far as I am aware, can be anyone not in the starting eleven. He does not need to be eligible to play Test cricket, and will not be named on the team sheet.

His duties are to carry the drinks, run errands for the players in the middle, and act as a substitute fielder (although often other players are used for this duty).

A substitute fielder is allowed when a member of the fielding side is injured or ill. However, he can only field, cannot bat, bowl, or keep wicket (although he can keep wicket if the opposing captain allows it).He is also not allowed to captain the team he is acting as a substitute for.

  3. Twelfth man

Traditionally, the first substitute player who fields when a member of the fielding side is injured. In Test matches, twelve players are named to a team prior to the match, with the final reduction to eleven occurring immediately prior to play commencing on the first day.

This gives the captain some flexibility in team selection, dependent on the conditions (e.g. a spin bowler may be named to the team, but omitted if the captain feels that the pitch is not suitable for spin bowling).

I think you should read the Link below that says "uncyclopedia" ;  Says how 12th man is  used, l0l. Very Amusing!

  4. If he is chosen as twelfth man he is already eligible for a full test.
