What is wrong with my skin?

I have patches of white on one of my elbows and on the back of my legs.

Sometimes when i sweat it gets all read and itchy or flaky.

I've had it for about 4 years and its spread to my other elbow.

What is this called and how can i make it go away.

PS. I have used many different fungal creams but nothing seems to work.

Maybe a doctor can prescribe ketoconazole (Nizoral).
