well, what can I say, they can do all medical practice that you can imagine: surgery, dentistry, facial lifts,.. and their tool.. huh! just Saliva.. what can I say...

Internist and Generalist doctors are mostly needed because the wide number of different diseases by the masses is more than the capacity of the present doctors now.. Plastic Surgery is also becoming a popular and hot career especially that it serves the upper middle class to the elites..

It just depends on who you wanted to serve more and how much money you wanted to make

Generalist/Internist - makes less but serves more especially the poor

Plastic Surgeon - earns more but serves only a few percentage of the population usually belonging to the upper 30% of the population in the upper class bracket

...we do have a high birth rate so i would say the obstetrician (OB) which could be a general practitioner.  in some areas, the midwife (comadrona) still does brisk business.
