when I flew from the US to the UK in 2002 I filled out a card on the airplane.  I think (and could be wrong) that it was an I-95 form.  My husband did the same thing when he flew from the UK to the US.  

He stayed 5 weeks, and I stayed 14 months (I did have to extend mine)
