What kind of medical insurance do competing MMA fighters get?

Question

Given that MMA is a hazardous sport... What would a typical policy (coverage, premiums, etc) look like? Do you know of any insurance companies that specialize in ensuring fighters?

Depending on the organization, some promoters are required to carry there own insurance for the fights.

I've included a link to a youtube video with Tito Ortiz where it is said that no fighter in the UFC is provided with any kind of insurance through the organization.  Dana White claims it is too difficult to insure his fighters.  It happens at the 3 minute mark.

maybe they just have to pay out of pocket.
