  1. A bunch of questions relating to your medical history, your plans for your pregnancy and breastfeeding.  They also ask demographic type questions, age, education, ethnic background, whether you use drugs, alcohol or nicotine.  Just basic stuff that a nurse would ask you at a doctor's office.

  2. Really they just ask for your checks/income, due date, do you have a doctor? They ask for your social security number and birth certificate. They ask who do you life with. They will discuss breastfeeding and your plans for it. If you haven't had your baby they will take a blood sample to check your iron I believe, and ask/talk to you about your eating habits. It's really not too difficult and a great program to be on.

  3. How far along you are.

    When is your due date.

Address (bring proof of address besides license or ID).

    Phone number

Someone you also want to add to pick up your WIC if you can't or don't want to go to the store.

    Proof of income.

Pretty much its just basic info.

They will also weigh you and prick your finger to check your sugar level.

  4. They ask if you or your child are on medicaid, who lives in the house, I think the ages of the people in the house, the combined total income for all members of the household, child's primary doctor, the height and weight of the baby at birth (if you've already had the child) and if you haven't had the baby yet, I do believe you have to take proof of your pregnancy, along with your due date.

  5. They will look at all your paperwork to verify your income, ask you about eating habits of yourself and children, check your height and weight, and also test your blood to see if you have anemia (just a quick prick on the finger).  It's pretty straight-forward and simple.

  6. Typically just some general questions about your health.  If your pregnant, they'll ask you how far along you are, what your lifestyle is like (i.e. make sure you don't need assistance getting off drugs, smoking, drinking, etc).  Beyond that, most of it is about what you eat to make sure you are getting all the necessary food groups and vitamins.  They also weigh you and measure your height, then they track it during your pregnancy and postpartum time.

  7. What they mostly care about is how much household income you have.  You have to meet the requirement first.  You also have to do a hemoglobin test (for you if you are pregnant, for your child if the WIC is for him/her) to see if your iron is sufficient.  If you are on Medicaid for pregnancy, you automatically get accepted into WIC.
