What kind of surgery do they do for an A/C separation, in the right shoulder, of the third degree ? Do they scope it, or do they actually make an incision and go in ?

  1. Yes.  The joint is so close to the surface it is sort of a mute question.  With a scope they have to make a tiny incision to put the scope through anyway, so ...

But OUCH!  I hope they take good care of your shoulder.  Be very careful to immobilize then do your range-of-motion and rehab exercises EXACTLY as instructed so you can have an optimal recovery.  DON'T let your shoulder freeze up!

Here are a couple of references, but the actual technique will depend on the surgeons' training and preferences.

Don't get scared by what it looks like in the references.  Your orthopaedic has lots of info for you, I'm sure, and it will be more reliable than the web documents.
