What leads to the breakdown of immunological tolerance?

I under stand there's 3 forms, 1)central tolerance, 2)peripheral toleranceand 3) acquired ttolerance But iI'mnot sure what aactuallyleads to it.

AALSO What are the characteristic features of the immune ddisease

  2. In one of these 3 conditions, immunological tolerance will breakdown:

    1.Exogenous antigen has the same epitopes with surface antigen in body.  

    2. The release of antigen in immunologically privileged sites.(e.g If one eye breaks up and the lens release out, the other eye will be defined to be a foreign body and then become blind as well.)

    3.The immune system has drawbacks, which deter the production of lymphocyte.

    The characteristic features of immune disease:

    1.Autoimmune antibody and autoreactive T lymphocytes can be found in the body of the patient.

    2.T cell mediate acquired immune response to self cell or tissue, which cause trauma or dysfunction.

    3.There are a definite link between the outcome of the disease and the intensity of  autoimmunological reaction.

    4.The seizure of disease is repeated, and chronic.
