  4. I don't know if requirements vary state by state, but I would imagine they are pretty similar.  Where I live you need:

    to weigh at least 110 lbs,

    to be18 (17 w/ parental consent)

    to be healthy (this includes a variety of things from no colds to no cancer)

    to not be on certain medications  

    Plus there is a long list of questions regarding your sexual history & where you have traveled, that is designed to screen out people at high risk for HIV & other problems that can be communicated via the blood.
