  1. It's fairly small around 1-2 percent.

  2. About 1.7%, mostly chinese and koreans, but also brazilians, filipinos , americans and europeans.

  3. 0%

  4. "In 2003, there were 1,915,030 registered foreigners in Japan, representing 1.5% of Japan's population of 127 million."

I'd say "the three major nationalities of residents in Japan who are not Japanese citizens" are:

    Koreans (32%), Chinese(24%), Brazilians, Filipinos(10%)...

    (% According to the link.)

I live in Tokyo and I'd say that Tokyo has the highest percentage of foreigners, I (American) being one of them.

Take a look here, the data is three years old, but it'll give you an idea:

    Hope this helps.

*Perhaps someone will post a current info link.

  5. In Tokyo, around 10% is non-Japanese, but in small cities, about 100% is Japanese, so in total 5% is non-Japanese.

The majority is Chinese, Korean, South East Asian and Brazilian for illegal immigrants, and American, Australian and British for English teachers, legal workers.
