well, there have been lots of different publishings of this book.. so some are like hardcover and others paperback.. so it really depends on what kind of to kill a mockingbird book u have right now.  but if you're wondering who said it, it's miss maudie.

i'm not sure, but i know its early in the book and ms.maudie says it to one of the children after they ask her why Atticus told them not to shoot a mockingbird. Sorry I cant help more. add a detail of the publisher of your copy of the book and you'll get more accurate answers.

Depends on what version of the book you have, but in mine, it's on page 90 in the middle, so it should be around there...
