What paperwork do i need to bring to my WIC appointment? i wrote it down but can't find the paper?

all i can remember is the immunization record, proof of address, my license, and they probably want the babies birth certificate and soc card

  1. Everything that you listed plus proof of income or proof that the child is on medical card. Since it takes so long for birth certificates and social security cards to come in the mail, they usually accept the Hospitals Record of Birth and the baby's hospital bracelet. Good luck and congratulations.


  2. If you don't have a job, they'll want proof of that.  Maybe bank statements.  I think that about covers it.

  3. From what I remember, that's all. Of course, you need to bring the baby also. If you're worried about it though, just give them a quick phone call before you go to the appointment!  Good-luck! WIC has been a lifesaver for our family! Formula is SO expensive!

  4. I'd take copies of your lease, if you're renting a house or apartment and copies of your monthly bills and bank statements.

  7. thats everything:

    immu, pay stubs, proof of address (license), child, any doctor forms or diet questionnaires.  If your baby was first born, then you will need his bed card, if not then the birth certificate.  I dont believe they need the social but bring it just in case.
