  2. You need the number for your local tax office, you should find it online under inland revenue.

  3. It is unlikely you will need a form for this, but depends on what sort of income from which tax has been deducted/paid.

    If you are self-employed, any refund will be made when your tax return is received and processed. A refund will not be considered before.

    If you are employed, just send form P60 to tax office named thereon, and ask for computation and refund. If any other information is needed, tax office will advise you.

    If you are reclaiming tax on interest received, you will need form R40, which you can download from www.hmrc/r40.
