What position do you put a patient in with emphysema?

Since they have been stupid enough to smoke and are now dieing from lung damage they should have their head elevated some, they will be lieing down soon in their coffins. As a matter of fact, you will know when they are about to die because they will INSIST on lieing flat shortly before they die.
