What proof of Service is required from a Process Server in GA .....?

I am in GA.What form of proof is required from a Process Server that they served a Summons on someone?Does it have to be a notarized affadavit or can it be as simple as taking a copy of a Sherriffs Entry of Service form and chaning it to just state Entry Of Service and where it is signed just typing Process Server under the signature without being Notarized?Would this be acceptable in Court or must it be Notarized in Georgia?

  1. A process servers signature is all that is required, they can also post to the premises after they make an attempt to serve and sign the form with the date it was posted and complete the service in that fashion.~
