  3. Here in SoCal in school we have earthquake drills (similar to fire drills) where we teach the students to duck and cover.  It means get under a desk and protect the head.  When the shaking stops, evacuate the building as you would during a fire drill.

  5. If you are in bed, the best thing to do is get to the nearest doorway, they are reinforced and will provide more stability in an earthquake. When I was little we were taught to get under a table, but I heard that is not taught anymore due to the possiblity of collapse. You are not supposed to run, or be near windows, or walls with picture frames. Precautions are still the name of the game. Make sure your water heater is securely fastened, all childrens furniture should be fastened to the wall. (small bookcases and such) and of course, keep standard supplies on hand. (food, water, batteries, battery powered radios)
