What should I included in a teacher-parent agreement?

I'm start my private teaching and I know many teachers have their students and parents sign a simple agreement or contract...what should I included in it? Thxs!!!

Include your entire syllabus

Make SURE you outline make up work and whether or not you accept LATE work (which is not the same)

your grading scale and how you calculate grades

the mandatory "cheating" discussion - be specific

your classroom management plan (what happens on a first offense, second offense, etc.)

any other thing that might come into question during the course of the year

make sure there's a place for the student AND parent to sign
