What should a middle age woman wear to a doctor or health care provider?

Yes, if she's too ill or incapacitated to dress.

Otherwise she can get dressed, go to the Dr., then disrobe to be checked.

I think if she is in pain and really sick, yes. Some people hurt too bad to wear pants or have any kind of elastic squeezing them. If the person is ok and can dress themselves, then no, dress appropriately as you would dress for any appointment.
