What states do you have to fill out forms in order to be online schooled?

I live in Utah, and i read somewhere that in some states you have to fill out forms, in others you don't and you can do it right away (or something like that.). So what states do you have to? Do you have to in Utah?

You have to at *every* public online school...private ones, too.  All schools have enrollment paperwork.

It doesn't matter if you use a public or private online school - you're going to have enrollment forms.  Most require your birth certificate, SSN, vaccination records, etc.
