What status will i declare on my visa application if im married based on NSO records in Manila?

I had this civil wedding in 1995 but unfortunately my groom abandoned & left me & didnt live with me.Marriage was held in Quezon City hall, Philippines.I did not use his name & all of my transactions & documents is still on single status.Now that i want to travel & go to the states to take a vacation, what status will i tick on my application?Is it single or will I declare it as married?My fear is that if i will put on single, they will check it through NSO & will find that I have a record there as "married".If i will declare it as married all of my documents are under my maiden name.My marriage has not yet been annulled nor a legal separation hasnt been filed yet.My ex already has a family of his own & i too has my own son from a past relationship.Please help all i want to do is to travel & have a grand time. Thank you

  1. Married.  For God's sake, don't lie when dealing with the US embassy in Manila or you'll find yourself banned from travel to the US for life.

You have had 12 years to address the situation... maybe it is time for you to follow through.  Until then, you are legally married - the records exits.  Declare your status as being married, and add comments if you wish; you will be provided the opportunity to add to the explanation if necessary.
