What steps does a U.S citizen need to take to get a passport?

My husband is a U.S citizen and and we are going on a cruise this November. He has never had a passport. What specific steps does he have to take to get one? How much? How long will it take for him to receive it?

He needs an official birth certificate

two photos official passport size

personal check for required amount

application filled out       go to your post office to apply

Cost @ $100  may take up to 12 weeks to receive it

It only takes a few weeks and not much money. He needs to go by the local passport office with his birth certificate and driver's license. He also needs a passport which is a different office. Passports may cost $130 or so and take less than a week to get.
