What two medications taken together would surely cause a heart attack?

Sorry, I'm asking quite a few questions lately to try to plan out this next chapter in my story. >_<

Are there any medications that, when taken together, will cause a heart attack? If so, what are they? And in what dosage would this be pretty much guaranteed to work? I heard that Nitrates and PDE5 inhibitors taken together might cause a heart attack or stroke--is this true? And what's the estimated time from consumption until the heart attack takes place? No medications are off limits (for treating absolutely any ailment/condition, I don't care what it is) .

Please note that this is for use in fiction purposes only. (Hence me also needing to find a condition for my character).
