What would you do in the following situation?

You opened your mailbox and there is a letter addressed to you in fancy script with no return address. You open the letter and find a form accompanyed with the following note:

Dear (your name here),

It is time for the evaluation report of the higher power. Please fill out the accompanying form and rate the higher power on a scale of 1-10 (1 being horrible and 10 being above and beyond expectations). Please fill it out honestly as your response will dictate future actions of the higher power.

The form has the following labels with a 1-10 scale next to them:

Attendenance

Customer Service Skills

Attitude

Communication Skills

Overall perfomance

There is a small box at the bottom with the title "any additional comments" above it.

How do you fill out this form?

I would simply write on it

Dear The Unknown

The Higher Power is well aware of  how I feel they are performing.  Quite frankly my opinions of the Higher Power are between men and The Higher Power.  We have a personal relationship and would like to keep it confidential.

"You response will dictate future acttions of the higher power." First i would think, 'there's something not said here' and then try to figure out whatever i could about these 'higher powers' I wouldn't fill out the form until i knew what they were about and if i agreed with them...

Or i would put under 'Any Additional Comments'

It's junk mail, a joke, or a mistake. Throw it away. If you do you might get a correct one later if it is seriouse.

Any mail sent to me without a return address is never opened promptly gets thrown away. I don't have time to deal with the suspicious, stupid and potentially deadly shite the envelope contains.
