the boy falls off a fence. I read this as a child and nobody seems to know this book. Do you? I hope so!

Oliver maybe? That's the only story I know about a boy in an orphanage. or maybe it's Pinnochio and the caterpillar you're thinking of is actually Jimminy Cricket? lol.
